    24 Hrs a day, 7 Days a week.     

    Call as soon as possible after the incident. You will be asked to provide information pertinent to the claim including:

    • Social Security Number of the injured person.
    • Name, Address and Telephone number of the injured employee(home & work). 
    • Name and Telephone number of the Supervisor.
    • Department name.
    • Date and time of incident. 
    • Description of incident (how, where, why?)
    • Type of injury (cut, scrape, burn, etc.)  Minor injury log Adobe Reader.
    • Body part injured.
    • Witnesses (name, address, phone no).
