Criteria 1A: Qualifications of firms including the team members assigned to the project. (50 points)
Criteria 2A: Knowledge and past experience of similar type projects. (20 points)
Criteria 3A: Past performance of the firms. (20 points)
Criteria 4A: Amount of work awarded and paid by the County. (5 points)
Criteria 5A: Ability of team members to interface with the County. (5 points)

All requests must be in writing the A&E Consultant Selection Coordinator and either mailed to the department, e-mailed, or faxed to 305-375-1083. The requests must also be copied to the Clerk of the Board and their fax # is 305-375-2484, or refer online to the Cone of Silence Report.
What affidavits are required to be completed for a typical contract?
In accordance with Ordinance 07-143 amending Section 2-8.1 of the Code of Miami-Dade County, effective July 1, 2008, Vendors are required to complete a new Vendor Registration Package, including a uniform affidavit packet (Vendor Affidavits Form), before being awarded a new contract.
