Mail-In Redetermination for Child Care Services
Mail-In Redetermination: process provides an efficient and expeditious means for parents to continue to receive Child Care services. If your child care service was authorized for at least six months you are eligible to participate in the mail-in re-determination process.
A Mail-in Redetermination packet will be mailed to you 45 days prior to your last authorized day of services. The packet consists of complete instructions and forms required to complete the process. You are required to mail your child care packet by US Postal Services.
You must provide proof of residency
A copy of one item from the following list is an acceptable documentation for proof of residency:
- Driver’s license
- Utility bill
- Water bill
- Electric bill
- Land line telephone bill
- Cable bill
- Credit Card bill
- Rental agreement
- Mortgage statement
- Property tax bill

Complete the entire Packet and Mail to:

All forms and requested documents must be received by the due date indicated in the packet, by mail or in the drop box located at:

Please Note: If you drop the Packet off at the Martin Luther King Jr. Office Plaza Lobby it must be in the drop box before 11:00 a.m. on the last day of services. Failure to return the complete Packet prior to the last day of services may result in an interruption of services.
If your packet is incomplete or further clarification is needed, our staff will contact you. Once your packet is received and processed you will be informed in writing of your status for continue child care services. A copy of the child care status will be sent to the child care provider.
