A small amount of money was received from the State's Medicaid program ($500,000) to help approximately 400 low- to moderate-income Miami-Dade County residents cover a portion of their monthly health insurance premiums.
Miami-Dade Blue is a product of Blue Cross Blue Shield of Florida for both individuals and small groups (businesses) with less than 50 employees -- designed in collaboration with Miami-Dade County under the leadership of Commissioner Joe Martinez. The plan began coverage on July 1, 2009 and targets the 600,000 residents countywide under the age of 65 who are unable to afford private health insurance and do not qualify for public programs, such as Medicaid and KidCare. The program includes primary and specialty care, inpatient services, diagnostic services, and pharmacy benefits. It incorporates chronic disease management and emergency room alternatives to increase healthcare efficiencies, lower the cost of charity care, and ultimately improve health outcomes.

What is the Miami-Dade Health Insurance Assistance Program
The Miami-Dade Health Insurance Assistance Program will cover a portion of the monthly insurance premium for low- to moderate-income individuals enrolled in Miami-Dade Blue. How do I qualify for the program?
You must:- Be an individual 19 - 64 years
- Be a Miami-Dade County resident
- Be a US Citizen or Qualifying Non-Citizen
- Be Insured by Miami-Dade Blue individual health insurance (as licensed by the State of Florida and operated by Blue Cross and Blue Shield of South Florida)
- Not be eligible for Medicaid, Medicare, or any other public funding
- Must receive medical care at participating Federally Qualified Health Center
- Have an annual household income between 100%-300% of FPL (federal poverty level):

What’s next, if I initially qualify for the program?
You will be directed to a Blue Cross and Blue Shield of South Florida licensed agent who will help you complete an application for Miami-Dade Blue health insurance. If you qualify for this insurance, the Health Insurance Assistance Program dollars will be used to decrease the amount you pay towards your monthly premium.How much will I have to pay?
In addition to a one-time $30 premium deposit that is refundable if you do not qualify for Miami-Dade Blue (a BCBSF agent will offer other low cost options that may be of interest), you will be responsible for a portion of your monthly premium. To determine how much you pay, subtract the total monthly premium from the amount that you qualified. This balance will be your monthly payment responsibility. Program participants will be given coupon books with directions about these monthly payments.Have a monthly household income between 133% - 250% of Federal Poverty Level
- Income ranges for a single (1) individual is between $10,890 - $32,670
- Income ranges for a family of four (4) is between $22,350 - $67,050

What happens when all slots are filled at the FQHCs?
Individuals at FQHCs will be told that enrollment capacity has been reached and they will go on a waiting list and be notified if a slot becomes available. The waiting list will be kept for persons who meet the financial qualifications. A limited number of persons will be kept on the waiting list due to fund availability.
When will I know when program enrollment and wait list closes?
All callers will receive an updated message from 3-1-1, on OCHP voice mail and on OCHP web page thanking them for their participation and interest but reflecting that the program has closed enrollment as of the closing date.
Will it be offered next year?
Currently, we do not know. However, it is our hope that Florida State's Medicaid program will continue to offer the program to Miami-Dade residents if success of the pilot can be shown and there are sufficient funds available. OCHP will be monitoring the program throughout. Please check the OCHP website next year in late spring for updates.
