Upon receipt of the completed Initial Complaint Intake Form, a Code Compliance Investigator will open a case file and contact you in order to begin processing your complaint. When the initial investigation is complete, you may be required to complete a Contractor Complaint Affidavit . The Contractor Complaint Affidavit will be used by the Construction Trades Qualifying Board to assess restitution based on verifiable damages supported by the documentation you provide.
If you have already submitted a Contractor Complaint Affidavit, or if you choose to submit a Contractor Complaint Affidavit, you do not need to submit an Initial Complaint Intake Form. In this case, you will be contacted by a Building Code Compliance Office Investigator after preliminary review of the affidavit.
