All individuals seeking to influence legislation or the actions or decisions of Miami-Dade County's Board of County Commissioners, are required to register as lobbyists with the Clerk of the Board (COB) within five days after being retained by a principal and each year thereafter. Each lobbyist must pay a $490 lobbyist registration fee upon registering with the COB.
The forms that must be completed by all lobbyists and submitted to the COB pursuant to Section 2-11.1(s) of the Conflict of Interest Code of Ethics Ordinance are briefly described below.
The 2015 Annual Lobbyist Registration Form must be completed, signed and attested by a Notary Public. This form and the Lobbyist Activity Authorization Form must be filed with the COB before engaging in any lobbying activity.
The Lobbyist Activity Authorization Form serves as proof that the lobbyist is authorized to lobby on behalf of a principal. In addition to the lobbyist registration form, this form must be completed and signed by the individual retained as a lobbyist or by the principal.
The Joint Contingency Fee Affidavit states that the principal has not offered — nor has the individual(s) retained as a lobbyist, agreed to accept any contingency or success fee as defined in Subsection 2-11.1(s)(7) of the Miami-Dade County Code. This document must be signed and filed jointly by the principal and the lobbyist.
Notice of Withdrawal
This form states that an individual is no longer retained by a principal as a lobbyist and thus is no longer authorized to represent the principal. The Notice of Withdrawal must be filed with the Clerk of the Board of County Commissioners on the effective date of the withdrawal.
Each lobbyist must submit a signed statement ("Activity Expenditure Report") with the Clerk of the Board (COB) on or before July 1st of each year, pursuant to Section 2-11.1(s) of the Conflict of Interest Code of Ethics Ordinance. This report must be filed under oath, and provide a detailed listing of all lobbying expenditures for the preceding calendar year in excess of $25. A statement need not be filed if there has been no expenditures during the reporting period.
Annual Lobbyist Registration for Vendor Representative
Pursuant to Section 2-11.1(s) of the Code, all Vendor Representatives seeking to sell, market or influence a decision to purchase any product or service with Jackson Health System (JHS) funds or County Funds, must file Annual Lobbyist Registration Forms with the Clerk of the Board.
Notably, this form must be filed by Vendor Representatives before engaging in any conversation or verbal or written communication with a County employee or a JHS employee or medical staff member.
