New Fees Take Effect October 1, 2011
Copies of Business Affairs & Consumer Protection Division (BACP) licenses, permits and registration forms are available here in Adobe Acrobat (PDF) format. They are listed in alphabetical order and according to the BACP division (Consumer Protection Division or Passenger Transportation Regulatory Division) which regulates the particular industry or business activity.
The first column names the industry or business and briefly explains why a license, permit or registration is required. The second column names the particular document. Click on the name to view or print the document. Using a Frequently Asked Question (FAQ) format, the third column offers a more detailed explanation as to why the license, permit or registration is required, provides guidance as to how to correctly complete the document, and gives contact sources of additional information.
You can fill out the forms online and print it (see detailed instructions), or you can print out a blank form to fill out by hand. Please be sure to attach any supporting documentation (copies, not originals) and send it to us by mail or fax.
