    Seniors that do qualify and belong to District 12 yet do not attend any of our senior centers must go to the following Vita Sites:

    Jorge Mas Canosa Center

    250 SW 114th Avenue
    Sweetwater, FL
    Wednesday – 6 pm thru 8 pm, Saturday – 10 am thru 2 pm
    No Appointment Needed

    **Must bring the following items:

    • Valid Social Security Card
    • Photo ID
    • 1099 SSA Form – Amount of Income they received last year – or multiply their monthly income by 12 months
