CERTIFICATION

Community Business Enterprise (CBE) / Community Small Business Enterprise (CSBE)/ Small Business Enterprise (SBE)

The following forms are used in evaluating firms for participation in Miami-Dade County's Business Enterprise certification programs.

Request New Certification Package This form needs to be completed by all firms applying for CBE/CSBE/SBE certification programs.

Certification Renewal Affidavit Use this form for required annual renewal.

Change of Status Use this form to request changes to current certification status.

CSBE Personal Net Worth_Affidavit This form is required to be completed by firms requesting CSBE certification. This form is submitted in addition to the Request New Certification Package.

Certification Application Checklist This document should be used to assist in compiling all documents required in the certification process.

Local Developing Business Program (LDB)

Request New Certification Package This form needs to be completed by all firms applying for the Local Development Business Program.

Certification Renewal Affidavit Use this form for required annual renewal.

Disadvantage Business Enterprises (DBE)

Disadvantaged Business Enterprise is a federal program that ensures equal opportunity in transportation contracting markets, addresses the effects of discrimination in transportation contracting, and promotes increased participation in federally funded contracts by small, socially and economically disadvantaged businesses, including minority and women owned enterprises. Miami-Dade County is a Unified Certification Program member and processes applications for DBE certification. Please download application at:  http://osdbuweb.dot.gov/DBEProgram/index.cfm and submit to DPM for processing.

Continuing Eligibility Affidavit Use this form for required annual renewal.
