Form 1, Statement of Financial Interests - 2011 Tax Year

Local Officers subject to Financial Disclosure by the State of Florida (and candidates for these offices) are required to file a Form 1. Additionally, county employees authorized to approve expenditures exceeding $20,000 (including electronic signature approvals in ADPICS and FAMIS) are required to file this form. -
Form 1F, Final Statement of Financial Interests - 2012 Tax Year

Individuals who are required to file a Form 1 are required to file a Form 1F no later than 60 days after leaving that position, unless another position with a Form 1 or Form 6 requirement is taken within the 60-day period. -
Form 6, Full and Public Disclosure of Financial Interests - 2011 Tax Year

Elected constitutional officers, and candidates for such offices, are required to file this form. This includes, but not limited to, circuit court judges, county judges, clerk of the circuit courts, sheriffs, and county commissioners. -
Form 6F, Final Full and Public Disclosure of Financial Interests - 2012 Tax Year

Individuals who are required to file a Form 6 are required to file a Form 6F no later than 60 days after leaving that position, unless another position with a Form 6 requirement is taken within the 60-day period. -

Individuals subject to Financial Disclosure by Section 2-11.1(i) of the Code of Ordinances for Miami-Dade County (including full-time employees, advisory board members, and municipal employees) are required to file this form.
