The Miami-Dade County Elections Department reminds voters the last day to request an absentee ballot by mail is Wednesday, October 29, 2008. Any registered voter can request an absentee ballot regardless of the reason. Voting has never been more convenient than doing so from the comfort of your own home.
Absentee ballot request forms are available on www.miamidade.gov/elections or by calling 3-1-1. The request must be in writing and addressed to the Supervisor of Elections, Miami-Dade Elections Department, P.O. Box 521550, Miami, Florida 33152.
The written request MUST include all of the following:

Printed name of voter

Current address
-
Address where the ballot is to be mailed (if different than residence address, must state why)

Date of birth
-
Date of election(s) for which an absentee ballot is needed

Signature of voter
If requesting an absentee ballot in person, voters must bring a picture ID to the Election Department’s Main Office at 2700 NW 87th Ave., Miami, FL; or the branch office Voter Information Center (VIC) at the Stephen P. Clark Center, 111 NW 1st Street, Suite #112, Miami, FL (located on the 1st Floor, West Lobby).
Absentee ballots must be received by the Elections Department no later than 7pm on Election Day, November 4, 2008. Absentee ballots can be mailed using a first class stamp, but must be received - not postmarked - by November 4. Ballots can also be dropped off at either location stated above.
Voters who are unable to request an absentee ballot in person may designate someone to pick it up or drop it off for them. The designee must present a written request from the voter and their picture identification.
