A Voter Information Card includes all identifying information, as well as precinct and other elections-related information.
A voter may receive a replacement Voter Information Card by providing a signed, written request to the Elections Department. Upon verification of registration, Elections will issue the voter a duplicate card without charge. In the case of a change of name, address, or party affiliation, the Department shall issue the voter a new voter information card.
What does it mean if I receive a new Voter Information Card?
A new Voter Information Card may be mailed out:

In response to changes you submitted to the Department (i.e. change of name, address, or party affiliation),

A temporary or permanent polling place change, and/or,
-
During a countywide mailer to voters from the Elections Department.
How do I request a replacement Voter Information Card

Please send a written request that includes your name, date of birth, signature, or complete a new Voter Registration Application form. As the original signature is required per Florida State Statute, we cannot take phone requests. You should receive your replacement Voter Information Card within three weeks.
