Is your Voter's Information Card lost or damaged?
Please send a written request that includes your name, date of birth, and your signature, or complete a new application form. As the signature is required per Florida State Statute, we cannot take phone requests. The replacement Voter's Information Card should be received within three weeks.
It is not mandatory that you bring your Voter’s Information Card with you to vote. However, you must bring current and valid picture identification. You will be asked to complete a provisional ballot if you are unable to produce acceptable identification.
