- Full Name (First, Middle, and Last)
- New Residence Address
- New Mailing Address (if different)
- Date of Birth
Telephone: 305-499-8363
e-mail: Webmaster
Fax: 305-499-8371
Moving to a new Florida county or to Miami-Dade from another county?
Be aware that a move from one Florida County to another requires a new Voter's Registration Application Form.
Name and/or Political Party Changes
Please note that a new Voter's Registration Application Form is now required for these types of changes.
To change your name and/or political party, State Law requires an original signature on the document. Therefore, NO faxed or E-Mailed changes will be accepted!
When you change your address, an actual residence address is required by law. If you provide any kind of mailing service address, PO box, or business address, the change will not be accepted, and can cause your registration to not be accepted and can cause your registration to not be processed in time for an election. PO boxes and other business addresses can be accepted as a mailing address.
Florida Voter's Registration Application Form
(Please note that we will continue to accept the previous version of the Voter Registration Application throughout 2008.)
