All accidents must be reported on a form prescribed by the State of Florida or Miami-Dade County , to Miami-Dade County Office of Elevator Safety, within five (5) working days following any such accident, in accordance with Florida Statute Chapter 399.125. Failure to timely file this report is a violation of this chapter and will subject the certificate of operation holder to an administrative fine, to be imposed by Miami-Dade County, in an amount not to exceed $1,000.

All reports of accidents on this equipment shall be reported to Miami-Dade County Office of Elevator Safety on forms provided by the Office of Elevator Safety.
