Prepare the application; include all requested Information, including name of owner, physical address of building, tax folio number of property, and the applicable master permit number issued by the Building Department in the city, village or township of jurisdiction.

Submit appropriate fee by check or money order, made payable to Miami-Dade Board of County Commissioners, in the amount determined by the fee schedule, which includes the fee established by the State of Florida, the plans review fee, and the fee for the first year Certificate of Operation. If the permit request is for expedited processing, include the fee in accordance with the schedule.

Submit elevator layouts in duplicate which show plan view and elevation view of installation, including details of the entrance design, and cab design.

Submit a set of architectural plans which depict plan views of ground floor layout, typical floor layout, special floor layout, top floor layout, and machine room layout. The architectural set shall include elevation views of the building and floor heights, machine room, and pit. The drawings provided shall be sufficient to determine access to machine room. Structural, electrical, mechanical, and plumbing drawings are not necessary to be submitted unless elevator details are shown on those individual drawings, such as air conditioning for the machine room, or cab, or emergency power, etc.

If complete, the application will be processed by depositing the fees, reviewing drawings, and inputting data into Elevator Tracking System (ETS) which will issue the permit number. Until ETS can be programmed to issue permits, a manual ledger system shall be used, using a manual permit numbering system. The manual system shall derive the permit number from the date of application and the sequence during that date. A permanent State serial number will be assigned. Alteration permits will employ the same method until ETS is programmed after which they will have a unique number assigned by ETS.

All drawings will be reviewed and stamped if approved. One complete set of those drawings submitted will be retained by the County for our permanent record, the other stamped sets are to be retained by the contractor. One complete approved set is to remain on the project site with the issued permit at all times, until a final inspection has been approved.
