    Apply for Housing

    Open registration for the 2008 Waiting List is now closed. Applications for project-based units (Public Housing, Moderate Rehabilitation and Section 8 New Construction) and tenant-based units (Section 8 Housing Choice Voucher) are taken only during periods of open registration. Selected applications will be processed at MDPHA's Applicant Leasing Center. For more information visit our Waiting List page.

    Required Documents (at the time of eligibility interview)
    All applicants undergo a housing eligibility interview. Applicants are required to bring the following documentation to the interview:

    • Picture ID, such as a driver’s license
    • Social security cards for all persons appearing on the application
    • Medicare and/or Medicaid cards
    • Proof of age and other documents related to family or household make-up including birth certificates, divorce and custody papers, citizenship, immigration papers, etc.
    • Proof of all income, including letter from Social Security, Veteran’s Administration, pensions, employment, and child support payments. The verification of Social Security benefits must be requested prior to the applicant’s scheduled appointment.
    • Proof of assets such as bank accounts, life insurance policies, etc.
    • Your doctor’s name, address, and telephone number
    • Names, addresses, and phone numbers of at least two people to contact in case of emergency
    • Your last three landlords’ names, addresses, and telephone numbers and copies of your last three utility bills
    • Verification of child care expenses
    • Verification of full-time student status for dependents over 17 years of age and record of attendance for all school-age children
    • If the head of household or spouse is over 62 years old or over, or a person with disabilities, you may bring verification of medical expenses or anticipated expenses such as, health insurance payments, on-going prescriptions, doctor’s visits, payback agreements on hospital bills, etc.

    Reasons for Denial of Housing

    • Eviction or termination from housing under the U.S. Housing Act of 1937 because of drug related activity within 3 years from date of eviction
    • Participation in violent criminal activity within 5 years of eligibility determination
    • Conviction of manufacturing or producing methamphetamine (permanent denial)
    • Is subject to a lifetime registration requirement under a state sex offender registration program
    • Has committed fraud, bribery, or other corrupt or criminal act in connection with any federal housing program within 3 years of eligibility determination
    • Termination of Section 8 assistance within 3 years of eligibility determination or if they are currently in the process of eviction
    • Previously lived in public housing or an assisted unit and vacated leaving an unpaid balance will not be offered assistance until the outstanding balance is paid in full or a repayment agreement for funds is reached with MDPHA
    • Refusal or failure to complete required forms or supply requested information or fails to allow verification of information shall be seen as non-cooperative and will render the application ineligible for selection
    • Misrepresentation of information related to program requirements, including but not limited to income, family composition, citizenship, and immigration status shall render the application ineligible
    • Income limits apply

    Updating Information on your Application
    After the application period is over you can submit changes to your application. If your address or family size has changed, the head of household can complete a Change of Address Adobe Acrobat Reader required or a Change of Family Size Adobe Acrobat Reader required form. If you have been denied housing and would like to exercise your right to an informal hearing, you must submit an Informal Hearing Request Adobe Acrobat Reader required form. Once completed, return the forms to MDPHA's Applicant Leasing Center.
