Pre-Employment Physical and Drug Screening
New Hires are required to undergo a Pre-Employment Physical which includes Drug and Alcohol Screenings. This appointment will be scheduled by the hiring department.
Completions of Required Forms
Prior to the "New Hire Processing Appointment," new employees are required to complete the forms below and bring them to their scheduled appointment.
New Hire Processing Appointment
Job applicants that have been extended an offer of employment will be advised by the hiring department of their scheduled new hire processing appointment.
Note: Applicants must bring the completed forms above and identification(s) to the appointment.
Provide Forms of Identification
During the "New Hire Processing Appointment," new employees must provide one or more of the following forms of identification using List A, List B or List C.

Fingerprinting and Background Check
At the time of the New Hire Processing Appointment, fingerprints will be taken of all new hires. This information will be used to perform a background check.
