    PIP Pre-approvals
    
    The regular business hours of the PIP are 8:30 a.m. to 5:00 p.m., Monday through Friday.  To assist caregivers (hospices, nursing homes, adult living facilities) with dispositions that may arise during non-business hours of the PIP, pre-approvals can be made with our office.
     
    Pre-approval forms (see below) may be completed by the adult care facility for any patient, client or resident who appears to be a candidate for PIP.  Once forms have been submitted and approved by the PIP office, the removal service will be notified and will be able to assist with removals during non-business hours.
     
    For cases that are not pre-approved, caregivers must contact a funeral home or a removal service for storage arrangements.

    Please select the appropriate set of forms and return to the PIP office (Fax: 305-545-2409).
     

    Cases involving next-of-kin should complete:

    1. Pre-approval Checklist
    2. Decedent Information Form (Adult or Infant)
    3. Financial Disclosure Statement and Authorization for Cremation and Disposition
      (either written or verbal)

    Cases involving no next-of-kin should complete:

    1. Pre-approval Checklist
    2. Verification of No Next-of-Kin Affidavit
    3. Cremation Authorization Form (either written or verbal – if there is a legal or personal representative)

     

    All cases should include a copy of the facility’s Admission Face Sheet.
