Care Planning
This Program assists isolated elderly persons 60 years of age and older, when requests for help are directly received from persons in need, their families, neighbors, or other agencies. Assessing the person's problems, counseling, and developing a care plan are done in the person's home with their input. Services include helping people who require crisis intervention and need in-home care and regular monitoring. Emphasis is given to assisting functionally impaired people, enabling them to avoid institutionalization.
