The Mail-In Redetermination process provides an efficient and expeditious means for parents to continue to receive Child Care services. If your child care service was authorized for three months or more you are eligible to participate in the mail-in re-determination process.
A Mail-in Redetermination packet will be mailed to you 45 days prior to your last authorized day of services. The packet consists of complete instructions and forms required to complete the process. You are required to mail your child care packet by US Postal Services.
A copy of one item from the following list is an acceptable documentation for proof of residency:
- Driver’s license
- Utility bill
- Water bill
- Electric bill
- Land line telephone bill
- Cable bill
- Credit Card bill
- Rental agreement
- Mortgage statement
- Property tax bill
All forms and requested documents must be received by the due date indicated in the packet.
Complete the entire packet and mail to:
