Your neighborhood Team Metro Office accepts passport applications Monday through Friday from 8 a.m. to 4 p.m. only. You may also apply at the Government on the Go Bus.
Passport Documentation Required:
In order to assist you more expeditiously, all documents must be originals and you must provide the following requirements:
1. Two recent identical photos, 2" x 2" with a white or off white background.
2. Proof of U.S. Citizenship (Original only)
- Certified U.S. birth certificate listing date recorded (long form)
- Naturalization Certificate
- U.S. Consular Report of Birth Abroad
- If deriving citizenship from U.S. parents: proof of birth certificate (translated and notarized, if from foreign country). Also, proof of sole guardianship (translated and notarized, if from foreign country)
3. Current picture I.D.
- Any other I.D. which contains a picture
Appropriate fees are submitted:
Passport Book
- For applicants 16 or older, $100 ($75 passport fee payable to U.S. Department of State + $25 execution fee payable to: Board of County Commissioners).
- For applicants under age 16, $85 ($60 passport fee payable to: U.S. Department of State + $25 execution fee payable to: Board of County Commissioners).
Passport Card
- For applicants 16 or older, $45 ($20 passport fee payable to U.S. Department of State + $25 execution fee payable to: Board of County Commissioners).
- For applicants under age 16, $35 ($10 passport fee payable to: U.S. Department of State + $25 execution fee payable to: Board of County Commissioners).
Expedite Process (up to 3 weeks). Add $60 per application.
Regular Process (up to 6 weeks)
All Team Metro Offices and the Government of the Go Buses offer $10 passport photo fee which you can pay in checks, credit cards, debit cards, cashier checks and money orders. The Government on the Go Bus cannot accept credit or debit cards.
Parents submitting applications for minor children must bring children's birth certificates to establish relationship.
Name changes need to be justified with marriage and/or divorce documents

Additional Information
- All applicants must be present.
- Applicants 0-16 must appear with both parents. When one of the parents is not present, parent or legal guardian must present consent application notarized from the other parent.
- Adult Passports (16 years old and older), are issued for 10 years
- Minor Passports (15 years old and younger), are issued for 5 years
