Individuals with a disability that prevents them from riding public transit can apply to be certified for the STS program. Please contact the Paratransit Operations office at 786-469-5000 for an application to be mailed to you directly.
Note: All questions on the application must be answered. You must also have a medical representative complete and sign the form before sending it to us. Incomplete and/or unsigned forms will not be accepted and may cause a delay in determining eligibility.

When sending your completed application by mail, make sure to include a letterhead or prescription form with the names and addresses of the medical representative and the applicant. In some instances, eligibility cannot be determined without further information. We recommend you contact your medical representative for additional medical documentation which substantiates your medical condition and provides insight into your abilities or limitations in using regular public transportation.
If you have no other means of transportation, STS will provide you with transportation to the in-person assessment at no cost to you. However, any companion attending the assessment meeting with you must pay the regular $2.50 STS fare one way.
