Premise Location Certification

Title companies, attorney’s offices, and residents requesting a document listing outstanding balances on a property please see the information below on Premise Location Certifications.

Ordering a Premise Location Certificate

A Premise Location Certificate (PLC) documents the Water & Sewer Department's interest in a property by listing liens or other financial responsibilities.

To order one:

  • Complete the PLC form 
  • Via fax at 786-552-8349
  • By mail or by dropping it off at:

Please note: In order to have your request processed by fax, you must have an established PLC payment account and prepaid the amount for this service.

Establishing an account

To establish an account, fax your request to 786-552-8349 and provide a contact telephone number for a representative to assist in establishing a PLC account.

Paying for a Premise Location Certificate

A PLC requires an advance payment of $20 per request for a 24-hour written response. The cost for a written response within 7-10 working days is $10 per request.

There are different ways to pay:

  • Establish a PLC account. We recommend that you establish an account from which payments may be drawn if you make numerous requests.
    • The Department will process each request in the order received not to exceed the balance of the account.
    • Fax requests by customers with an established account will be handled within 24 hours if received prior to 2 p.m. Monday through Friday. All requests received after 2 p.m. will be ready for pickup the second business day after 12 noon.
  • Standard request. All PLC requests must be mailed to the address below, accompanied with payment and a self-addressed stamped envelope.

Properties not served by Miami-Dade Water and Sewer Department will be advised accordingly on each PLC. However, the Department does not provide written notice of service verification through the PLC process. Those customers interested in service address validation must contact the New Business Office at 786-268-5360.
