Work under this Contract includes the furnishing of all supervision, labor, materials, tools, equipment and performing all operations required to construct the Work in accordance with the Contract Documents. The overall goal of this project is to interconnect two existing canal systems. The Scope of Work includes but is not limited to the construction of a slab covered canal within the eastern edge of the existing canal right-of-way, which runs parallel, east of SW 157 Avenue. The southern terminus of the canal will be a connection to an existing slab covered canal at SW 59 Terrace. The northern terminus will be an outfall at the existing canal on SW 42 Street. Construction will include a headwall at the southern terminus of the existing slab at SW 64 Street and SW 157 Avenue and a headwall at the northern terminus of the new canal. Both headwalls will be outfitted with manatee gates and sodding will be placed over the final slab covered canal. LOCATION OF WORK: The location of work to be performed under the terms of this Contract shall be as follows: SW 157th Ave from SW 64th St. to SW 42nd St.
Technical Certification :
LICENSE REQUIREMENTS: At the time of Bid and pursuant to the requirements of Section 10-3 of the Code of Miami-Dade County, Florida and these Solicitation and Contract Documents, the Bidder must hold a valid, current, and active: a. Certificate of Competency from the County’s Construction Trades Qualifying Board as a General Engineering Contractor or as a Specialty Engineering Contractor, commensurate to the requirements of the Scope of Work, in one or more engineering crafts to include pipeline engineering contractor or paving engineering contractor. The specialty contractor shall subcontract with a qualified contractor any work which is incidental to the specialty but is specified in the aforementioned Code as being the work of other than that of the Engineering Specialty for which certified; or b. Certification, as a general contractor or as a underground utility and excavation contractor, provided by the State of Florida Construction Industry Licensing Board, pursuant to the provisions of Section 489.115 of the Florida Statutes. EXPERIENCE REQUIREMENT: The Bidder, through full time personnel employed by the Bidder, must demonstrate a minimum of five years of continual experience as the prime contractor in projects with scopes comparable to the Project Scope of Work. Demonstrate the experience requirement by: a) Providing a detailed description of completed projects, similar to the Project Scope of Work, in which the Bidder is currently engaged or has completed within the past ten years. List and describe those projects performed for government clients, similar size private entities, and any work performed for the County. The description should identify for each project (1) the client, (2) description of work, (3) total dollar value of the contract, (4) contract duration, (5) customer contact person and phone number for reference, (6) statement or notation of whether Bidder is/was the prime contractor or subcontractor, and (7) the results of the project. INDEMNIFICATION AND INSURANCE REQUIREMENTS The Contractor shall furnish to Public Works and Waste Management Department, 111 NW 1 Street, Miami Florida 33128, Certificate(s) of Insurance which indicate that insurance coverage has been obtained which meets the requirements as outlined below: - Worker’s Compensation Insurance for all employees of the Contractor as required by Florida Statute 440. - Commercial General Liability Insurance on a comprehensive basis in an amount not less than $1,000,000 combined single limit per occurrence for bodily injury and property damage. Miami-Dade County must be shown as an additional insured with respect to this coverage. - Automobile Liability Insurance covering all owned, non-owned and hired vehicles used in connection with the work, in an amount not less than $1,000,000 combined single limit per occurrence for bodily injury and property damage.
