The scope of work consists of installation, labor, permitting of required hardware, testing configuration, installation of network fiber optics cable, replacing existing connectors, the enclosure of cameras with mounting brackets at varied Metromover stations, and integrating the entire system. (REFER TO FULL SUMMARY WORK) MDT MAY SELECT TO EXCERCISE THE FOLLOWINGS PROJECT OPTIONS: CCTV PROGRAMMABLE LOGIC CONTROLLER REPLACEMENT & NETWORK SCADA WIRELESS NETWORK VOIP TELEPHONY EQUIPMENT INSTALLATION AND LICENSING 1. Sealed Envelopes (only) with the Department Name project name, project number and the Number of the Request for Price Quotation (RPQ) noted on the outside of the envelope 2. Number of Copies of Bids: Three (3) one (1) original and two (2) copies 3. Bid Bond Required: YES totaling 5% of bid submittal price 4. Payment and Performance Bond required: YES 5. Forms of Payment: Cashier's Check, Certified Check, Money Order or a Company Check Payment must be Made Out to: Miami-Dade County Board of County Commission NOTE: Cash payment will NOT be accepted. 6. Where to Submit Bids: All bid submittals must be sent to the attention of: Office of the Clerk of the Board At: 111 NW 1st Street, 17th Floor Miami, FL 33128 (by 2:00 p.m. on the submittal date) NOTE: Late Bids submittals will not be accepted. 7. Last Day to Purchase Bid Documents: 5/9/2013 8. Cost of Bid documents: - Hard copy of Specification and Plans: $600.00 - CDs – Specification/Plans on CD: $10.00 9. Funding Source: 100% AMERICAN RECOVERY AND REINVESTMENT ACT (ARRA) 10. Davis Bacon Wage Decision will be enforced on this project and is located within the Bid Specification Package. 11. DBE Goals on this project is: 15% 12. Where not otherwise precluded by State or federal law, the overall per hour rate shall be the higher of the Responsible Wages and Benefits, pursuant to Section 2-11.16, Miami-Dade County Code or the rate of wages (including fringes) to be paid under the requirements of the Davis-Bacon Act; provided, further, that the overall per hour rate shall not be the higher rate if the federal government requires the County, as a condition of receiving federal funds for a project, to pay no more than the wages as determined by the U.S. Department of Labor under the Davis-Bacon Act on project contracts 13. DEADLINE FOR RECEIPT OF RFI QUESTIONS: 5/9/2013 14. This Request for Price Quotation (RPQ) is for a Miscellaneous Construction Contract. 15. Miami-Dade County, at its sole discretion, may elect to negotiate with the apparent low bidder. 16. Submittal of Federal Affidavits The Bidder is required to submit the following federal affidavits with the bid package: • Buy America Certificate • Certification Regarding Debarment, Suspension and Other Responsibility Matters • Lobbying Certification • Statement for Loan Guarantees and Loan Insurance • Disclosure of Lobbying Activities Failure of the bidder to complete and submit the above mentioned forms with the bid package may render the bid nonresponsive. 17. All terms and conditions of the MCC 7360 are part of this contract and will be enforced. Refer to Article 2.14 below of the MCC 7360 to further clarify the license requirements: 2.14 LICENSE QUALIFICATIONS OF CONTACTORS: A. All Contractors must hold a current valid Certificate of Competency for General Building/Engineering or Specialty Trades Contracting, as required by the Florida Building Code, for the types of Work covered by the Contract at the time of RPQ submission and maintain same throughout the duration of the project. The certificate(s) is to be issued by: 1. The State of Florida Construction Industry Licensing Board, pursuant to the provisions of Section 489.115 of the Florida Statute and registered with the Miami-Dade County, Building Department or, 2. The Dade County Construction Trades Qualifying Board, pursuant to the provisions of Section 10-3(a) of the County Code. Holders of Miami-Dade County Certificates of Competency must also hold Certificates of Registration issued by the State of Florida Construction Licensing Board, pursuant to the provisions of Section 489.115 or Section 489.117 of the Florida Statutes. B. Proof of such Certificate(s) must be submitted at the time of initial response and maintained current throughout the contract period. The County may request proof of continued certification at any time during the contract period. Failure to provide such proof within five (5) working days from notification by the County shall result in the removal from the contract and the rejection of any current or future RPQ bid submissions. C. Subsequent to the commencement of the Contract, the County may require specific qualifications based on a Project’s scope of work. Such requirements will be included within the RPQ.
