Contractor to permit and construct a one story CBS building, approximately 11,939 square feet, in accordance with the plans and specifications under Miami-Dade County Building Dept. process number C2012156588, prepared by Miami-Dade Fire Rescue Department (“MDFRD”), for a NEW MDFR Station 70, including but not limited to the following services: Site Clearing; Site Improvement; Earthwork; Soil Testing, Civil, Temporary Electrical Service; Electrical; Low Voltage; Fire Alarm; Mechanical (HVAC); Plumbing; Roofing; Interior Build-out; Floor Tile; Acoustical Ceiling; Cabinetry; Windows; Storefront; Stucco; Painting; MDFR Communications, Card Access Security and Coordination, Fire Alarm: Fire Protection; Emergency Generator; Solar Collectors; LEED Energy Management System (EMS); Waste Management LEED Collection; LEED Silver Requirements; CATV, FPL, WASD, PW, DERM, AT&T and any other Utility Installation & Coordination; Signage; Asphalt Paving and Stripping; Irrigation; Landscaping; Subterranean and Termite Control, for a complete operation of a Fire Rescue Facility. For Vendor bidding purposes a Geotechnical Report prepared by Ardaman & Associates as been provided within the specification manual. MINIMUM QUALIFICATIONS: Contractor shall have completed, at minimum, one (1) Silver or higher LEED Certification Building Projects, as a primary General Contractor or Building Contractor, within the last five (5) years from this RPQ’s issued date (2008-present). As part of the bid submittal package, Contractor shall provide a list of LEED Silver or higher Certification projects which meet the criteria above with the following by filling out the attached Minimum Qualification Form: • Project name: • Certification Level: • Project dates: • Contact building owner/representative name(s) & phone number(s): • Green Building Certification Institute ("GBCI") certification number: • Narrative of Work Performed: ADDITIONAL GENERAL REQUIRMENTS: A) Construction meetings will be held every two weeks upon project commencement. It will be the responsibility of the awarded contactor to have an authorized company representative, with decision making powers, at each meeting. Master permit and all other related trades are most important for the success of the project. Therefore a qualified contractor representative and/or supervisor able to make company decision according to the Miscellaneous Construction Contract 7040 shall be present at the job site during the duration/completion of the project. B) Master permit and all other relation subsequent permits are the responsibility of the award contractor and their hired sub-contractor, including shop drawing development and processing. Please review Section 2.24 of Contract - Permit/Inspections, Fees and Taxes. C) A construction security fence currently exists on the property and the use of it during the construction is at the discretion of the awarded contractor. Otherwise, contractor is to include, at minimum, at 6 feet construction chain link fence, gate and green screen secured at all times throughout the project duration. Contractors are to be aware of neighboring residence. D) Temporary toilets, storage containers, construction trailers and dumpsters are the responsibility of the awarded contractor, including removal and replacement prior, during and after a Hurricane Activation of “Watch and Warning”. E) The awarded contractor is responsible for securing the construction site at all times, including placement of all needed warning signs according to all regulation enforced by Authority Having Jurisdiction. F) The awarded contractor will not be responsible for geo-technical soil testing. During the project, MDFR will provide geo-technical testing services for the contractor to utilize at the request of the contractor. The contractor shall provide MDFR with a request for testing services with, at minimum, 3 days notice. Shall any testing fail inspection(s), the awarded contractor shall pay all costs associated with the re-testing(s). G) Test & Balance Inspection and Report: MDFRD will, at the end of the project, hire an independent testing company to inspect and perform specified Test & Balance Report. MDFR requires that the contractor complies with the requirements of the provided Test & Balance Report. If the T & B report fails, the awarded contractor shall pay all costs associated with the re-testing(s). H) Partial Payment request will include the following: Partial Releases from all vendors, suppliers, etc. (including those which have or have not furnished a Notice to Owner). Additionally, an updated Project schedule, updated Schedule of Values, Monthly Utilization Report, Certified Payroll and updated As-Built drawings must be updated frequently by all trades. Invoices will not be processed for payment unless As-Builts are updated. I) The awarded contractor is responsible for all processing and shop drawings development through the Miami-Dade permitting agencies, including all trades. Six (6) copies of Shop Drawings shall be submitted to the Owner (MDFRD) for approval, prior to submittal to the Miami-Dade County or State Agencies. J) The awarded contractor must comply with all the requirements set forth in the Miscellaneous Construction Contract MCC 7360 Plan - CICC 7360-0/08 , including all written directives established during pre-construction meetings and any other meetings part of this project. IMPORTANT: This project is covered under the Cone of Silence. Any questions pertaining to this project must be submitted in writing to the Project Manager. BID CD DOCUMENTS will be available for pick up beginning on 5/10/2013 at MDFRD HQ located at 9300 NW 41 Street Suite 237 between the hours of 8:00 a.m. and 4:00 p.m. at a cost of $25 per set. NON REFUNDABLE Certified Checks or Money Orders must be made payable to Miami-Dade Board of County Commissioners. Cash and personal/business checks will not be accepted. Bid CD documents will not be available for sale at the pre-bid site meeting. 2. SEALED BIDS for this project will be received for and on behalf of Miami-Dade County, by Miami Dade Fire Rescue at 9300 NW 41st Street, Suite 237, Miami, Florida, 33178 on June 19, 2013 @ 10:00 am. 3. LIQUIDATED DAMAGES in the amount of $500 per calendar day will be assessed for each day the project is delayed. 4. BID BOND: Contractor shall submit 1 original fully executed and 2 copies of original of a bid guarantee in the form of a certified check, cashier's check or bid bond in the amount of 5% of the base bid price, payable to the Board of County Commissioners of Miami-Dade County, Forida. 5. PERFORMANCE AND PAYMENT BOND: Bidders submitting Bids and to whom a contingent award is made shall duly execute and deliver to the County a Performance and Payment Bond in an amount that represents 100% of the Bid price offered by the Bidder. The Performance and Payment Bond Form supplied by the County shall be the only acceptable form. 00431 - Surety Performance and Payment Bond Form is herewith included. The completed form shall be delivered to the County within 14 calendar days of the date that the Notice of Award Recommendation is issued. 6. PREVAILING WAGES: Contractor shall obtain current Prevailing Wages from the Small Business Development (SBD). Please visit www.miamidade.gov/sba for contact information. 7. CSBE MEASURES: Contractor shall comply with CSBE goal of 24.13% for this RPQ. For your reference, sub trades identified by Small Business Deparment include: Mechanical, Electrical and Metals. 8. The last day for project RFI QUESTIONS is June 4, 2013 at 4:00 PM. Any questions submitted after aforementioned date shall not be considered. MDFR will review and answer the questions, prior to the bid due date. 9. ART IN PUBLIC PLACES: Awarded Contractor shall work with MDFR and AIPP should Art in Public Places be required following all the requirements stipulated in Section 2.11.15 of Miami-Dade County Code. 10. MANDATORY PREBID MEETING: A Mandatory Prebid Meeting has been scheduled on May 15, 2013 at 10:30 AM at Miami-Dade Fire Rescue Training Conference Room. Due to security checkpoints, please arrive at least half an hour prior to meeting time in order to avoid delays which may prevent Contractor from participation of Mandatory Pre-bid Meeting. 11. PRICE PROPOSAL: Contractor shall provide two base prices pursuant to attached Price Proposal Form. Contractor shall include all permit fees in each base price. Base Price A: shall include all work pursuant to RPQ/all addenda issued and the approved plans provided within this RPQ. This includes Terrazzo Flooring as indicated on the plans. Base Price B: shall include all work pursuant to RPQ/all addenda issued and the approved plans provided within this RPQ, with exception of the Terrazzo Flooring. “Pursuant to Miami-Dade County Budget Ordinance No. 03-192, this Contract is subject to a user access fee under the County’s User Access Program (UAP) in the amount of two percent (2%). All construction services provided under this contract are subject to the 2% UAP. This fee applies to all Contract usage whether by County Departments or by any other governmental, quasi-governmental or not-for-profit entity. From every payment made to the Contractor under this contract (including the payment of retainage), the County will deduct the two percent (2%) UAP fee provided in the ordinance and the Contractor will accept such reduced amount as full compensation for any and all deliverables under the contract. The County shall retain the 2% UAP for use by the County to help defray the cost of its procurement program. Contractor participation in this pay request reduction portion of the UAP is mandatory.” ADDITIONAL INSURANCES AS STIPULATED BELOW: INDEMNIFICATION AND INSURANCE Contractor shall indemnify and hold harmless the County and its officers, employees, agents and instrumentalities from any and all liability, losses or damages, including attorneys’ fees and costs of defense, which the County or its officers, employees, agents or instrumentalities may incur as a result of claims, demands, suits, causes of actions or proceedings of any kind or nature arising out of, relating to or resulting from the performance of this Agreement by the Contractor or its employees, agents, servants, partners principals or subcontractors. Contractor shall pay all claims and losses in connection therewith and shall investigate and defend all claims, suits or actions of any kind or nature in the name of the County, where applicable, including appellate proceedings, and shall pay all costs, judgments, and attorney’s fees which may issue thereon. Contractor expressly understands and agrees that any insurance protection required by this Agreement or otherwise provided by Contractor shall in no way limit the responsibility to indemnify, keep and save harmless and defend the County or its officers, employees, agents and instrumentalities as herein provided. The Contractor shall furnish insurance requirements for this RPQ to Miami-Dade County, c/o Miami-Dade Fire Rescue Department, 9300 NW 41st Street, Doral, FL 33178. 1. Certificate(s) of Insurance which indicate that insurance coverage has been obtained which meets the requirements as outlined in paragraphs A, B and C. 2. Original Policy which indicates that insurance coverage has been obtained which meets the requirements as outlined in paragraph D. A. Worker’s Compensation Insurance for all employees of the vendor as required by Florida Statute 440. B. Commercial General Liability Insurance on a comprehensive basis in an amount not less than $1,000,000 combined single limit per occurrence for bodily injury and property damage. Miami-Dade County must be shown as an additional insured with respect to this coverage. C. Automobile Liability Insurance covering all owned, non-owned and hired vehicles used in connection with the work, in an amount not less than $500,000 combined single limit per occurrence for bodily injury and property damage. D. Completed Value Builder’s Risk Insurance on an “All Risk” basis in an amount not less than one hundred percent (100%) of the insurable value of the building(s) or structure(s). The policy shall be in the name of Miami Dade County and the Contractor. This insurance coverage is to be maintained until substantial completion of the work as determined by the Miami-Dade Fire Rescue Department. Builder’s Risk Insurance: The Contractor shall include a separate line item for the required Builder’s Risk Insurance Coverage for the Project as shown on the bid form. The Contractor shall provide a detailed Builders Risk quotation including, Term, Carrier, limits, deductibles and pricing along with the bid proposal. The County reserves the right at its sole discretion to either: (1) Include the Builder’s Risk Coverage under the County Controlled Builder’s Risk Insurance Program or (2) Have the Contractor cover the project on a policy purchased by the Contractor. If the County chooses to include the Project under the County’s Builder’s Risk Program, the County shall be responsible for the Named Windstorm and the Flood deductibles and the Contractor shall be responsible for the All Other Perils deductible. If the County chooses to cover the project under the Contractor’s Builder’s Risk Insurance policy, the Contractor shall be responsible for all deductibles. All insurance policies required above shall be issued by companies authorized to do business under the laws of the State of Florida, with the following qualifications: The company must be rated no less than “B” as to management, and no less than “Class V” as to financial strength by the Best’s Insurance Guide, published by A.M. Best Company, Oldwick, New Jersey, or its equivalent, subject to the approval of the County Risk Management Division. or The company must hold a valid Florida Certificate of Authority as shown in the latest “List of All Insurance Companies Authorized or Approved to Do Business in Florida” issued by the State of Florida Department of Financial Services and are members of the Florida Guaranty Fund.
