This RPQ is for the project number Z000145 located inside the basement of the Dade County Courthouse (73 W. Flagler Street) Basement. The scope of work for this project consists of, and not limited to: demolition; concrete restoration and steel rehabilitation of the existing 140+ columns located in the basement; placement of a new floor slab; installation of a new waterproofing membrane system over the existing basement; and MEP. As part of the scope of work, the contractor will need to repair/reinforce/protect the existing steel columns, place temporary shoring, phase all of the construction, fire alarm and protection work, and have dewatering onsite. (See the project plans) Work for this project will be performed in the EVENING HOURS, with inspections performed by the City of Miami and/or Special Inspector in the daytime. The contractor will need to work closely with the Internal Services Department Project Manager, Courthouse Building Manager, and the Project Engineer for coordination and all project issues. The contractor qualifications for this project are, and not limited to: well versed in structural steel elements; experienced in working with construction materials dating back to the 1920s (including and not limited to: welding new steel elements to old 1920s steel material, riveting, etc.); have completed three (3) similar projects within the last eight years on buildings from the 1920s or earlier (with equal or greater building foot print); and minimum of 10 years in concrete restoration. The contractor as stated before will need to work mostly in the evening with non-disruptive work allowed in the day hours. Weekend hours will need to be coordinated will the ISD Project Manager and Building Manager. The project dry-run submittal has been approved by the City of Miami. ISD has obtained the Building Department permits; therefore, the Contractor will be responsible for ALL remaining and/or additional permit fees required, including and not limited to: hotworks, dewatering, sidewalk closures, etc. City of Miami permit Number: BD15-012967-001-B001. Comments: EMPLOY MIAMI-DADE PROGRAM In accordance with Section 5.02 of the Miami-Dade County Home Rule Amendment and Charter, Section 2-8.1 of the Code of Miami-Dade County, and Administrative Order No. 3-63, all contractors and subcontractors of any tier on (i) construction contracts valued in excess of one million dollars ($1,000,000) for the construction, demolition, alteration and/or repair of public buildings, or public works; or (ii) contracts or leases valued in excess of one million dollars ($1,000,000) for privately funded construction, demolition, alteration or repair of buildings, or improvements on County-owned land. The awarded Contractor is hereby notified that the County will consider whether the Contractor made its best reasonable efforts to promote Employ Miami-Dade on this contract, as defined in A.O. 3-63, as part of the County’s evaluation and responsibility review of the Contractor for new County contract awards. RESIDENTS FIRST TRAINING AND EMPLOYMENT PROGRAM In accordance with Section 2-11.17 of the Code of Miami-Dade County and Implementing Order No. 3-61, all contractors and subcontractors of any tier on (i) construction contracts valued in excess of $1 million for the construction, demolition, alteration and/or repair of public buildings, or public works; or (ii) contracts or leases valued in excess of $1 million for privately funded construction, demolition, alteration or repair of buildings, or improvements on County-owned land shall comply with the following: (i) prior to working on the project, all persons employed by the contractor or subcontractor on the project to perform construction have completed the OSHA 10-hour safety training course, and (ii) the contractor will make its best reasonable efforts to have 51% of all construction labor hours performed by Miami-Dade County residents. In accordance with Miami-Dade County Implementing Order 3-9, Accounts Receivable Adjustments, if money is owed by the Contractor to the County, whether under this Contract or for any other purpose, the County reserves the right to retain such amount from payment due by County to the Contractor under this Contract. Such retained amount shall be applied to the amount owed by the Contractor to the County. The Contractor shall have no further claim to such retained amounts which shall be deemed full accord and satisfaction of the amount due by the County to the Contractor for the applicable payment due herein. Pursuant to Section 2-8.10 of the Code of Miami-Dade County, this Contract is subject to a user access fee under the County’s User Access Program (UAP) in the amount of two percent (2%). All construction services provided under this contract are subject to the 2% UAP. This fee applies to all Contract usage whether by County Departments or by any other governmental, quasi-governmental or not-for-profit entity. From every payment made to the Contractor under this contract (including the payment of retainage), the County will deduct the two percent (2%) UAP fee provided in the ordinance and the Contractor will accept such reduced amount as full compensation for any and all deliverables under the contract. The County shall retain the 2% UAP for use by the County to help defray the cost of its procurement program. Contractor participation in this pay request reduction portion of the UAP is mandatory. Provided, however, UAP shall not be applicable for total contract values, inclusive of contingency and allowance accounts, of less than five hundred thousand dollars ($500,000.00). COMMUNITY WORKFORCE PROGRAM Prior to entering into a contract and according to the Miami-Dade County Code §2-1701 and amended by Ordinance 13-66, the successful bidder on a construction contract subject to a Community Workforce Program (CWP) goal, must submit to Small Business Development (SBD) through the contracting officer a workforce plan outlining how the CWP goal will be met. Additional information is available at the County’s website at http://www.miamidade.gov/business/contract-requirements.asp#0 . Contractor must submit a Workforce Plan to the Miami-Dade County Internal Services Department, Small Business Development Division within fifteen (15) days of notification of award of the contract. The County will not enter into the contract until it receives the contractor’s Workforce Plan and deems the Plan acceptable. The Workforce Plan forms may be obtained on the County’s website at http://www.miamidade.gov/business/contract-requirements.asp#0. Examination of Bid Documents: Before submitting a bid, the bidder shall examine the Bid Documents, which may be purchased from ISD/CAS at a non-refundable cost of twenty-five dollars ($25.00) for each complete set of documents. Payment shall be made in the form of a company check, cashier''s check, or money order payable to the Internal Services Department, Miami-Dade County, Florida. Bid documents consist of Attachment 5A (Bid Form), Contract Conditions and plans and specifications. All bidders shall become thoroughly familiar with all of the terms of this RPQ prior to the Pre-Bid meeting. Failure to do so will in no way relieve the successful bidder from completing the required work for the bid price. City of Miami permit Number: BD15-012967-001-B001. Bids Submittal: Bids must be submitted in a sealed envelope, and the bidder’s name and address, as well as the title and number of the contract for which the bid is being submitted, must be indicated on the outside of the envelope. The sealed envelope shall include the following documents: 1. Bid price using form 5-A, (provided in the bid package). 2. Bid guarantee in the form of a bid bond (using the form provided by Miami-Dade County), certified check, or cashier's check. Failure to include a bid guarantee shall render the bid non-responsive. 3. Vendor Affirmation Affidavit (If not previously submitted) 4. Internal Services Department (ISD) Division of Small Business Development’s (SBD) Form 400 “Schedule of Intent” (SOI) listing each one of the subcontractors to work on the project, If applicable. 5. Prospective bidders shall submit a minimum of three (3) references within the last eight (8) years that are representative to the general scope of work which is currently being advertised, and must also be of equal or greater value. References must include the following information 1) a description of scope of work preformed 2) project address 3) client contact information: name, address, phone number, and email address. Refer to the experience required under the scope of work. Bids will be received until 2:00 PM on the bid due date at the Office of the Clerk of the Board, 111 NW 1st Street, Suite 17-202, Miami, FL 33128. The sealed envelope will be opened shortly after the submittal deadline at 111 NW 1st Street, 18th Floor, Miami, FL 33128. Collusion Affidavit: Failure to provide a Collusion Affidavit within ten (10) calendar days after the recommendation to award has been filed with the Clerk of the Board shall be cause for the contractor to forfeit their bid/proposal bond.
