As an eligible Miami-Dade County employee, you may enroll yourself and eligible dependents for coverage in one of the three medical plans. The plans consist of one Point of Service and two HMO options.
A Point of Service (POS) plan allows you to receive services from an in-network or out-of-network provider of your choice. If you choose an out-of-network physician, your healthcare services will be subject to the plan deductible and co-insurance provisions.
A Health Maintenance Organization (HMO) provides a wide range of healthcare services to you on a prepaid basis. Under this plan, you receive medical services at no cost or for moderate co-payments without deductibles or claim forms.

Applies to AFSCME Water & Sewer, PBA Rank & File\Supervisory, and Transit Workers Union

Effective January 1, 2015, our new Pharmacy Benefits Manager (PBM) will be CVS Caremark.
This change will result in savings to our medical plan without affecting the pharmacy network and formularies. Your new AvMed member ID cards will indicate a new BIN (Bank Identification Number) to facilitate proper claim processing at retail pharmacies.
Important Note: Any mail order prescriptions you fill as of January 1, 2015 must be on the new CVS Caremark mail order form. You cannot use the old Medco form to order prescriptions after December 31, 2014.

The Select Plan
In 2015, we will be offering non-bargaining unit employees and bargaining unit employees, in accordance with their Collective Bargaining Agreements a NEW Select Option.

Members of the DCFF fire union may be eligible for coverage in their Union-sponsored plan. Contact your Union office for further details. If you enroll in the Fire Union-sponsored health plan, you may elect the Optix vision plan, but you cannot participate in of any County-sponsored dental plans.
