As an eligible Miami-Dade County employee, you may enroll yourself and eligible dependents for coverage in one of the offered medical plans.
A Point of Service (POS) plan allows you to receive services from an in-network or out-of-network provider of your choice. If you choose an out-of-network physician, your healthcare services will be subject to the plan deductible and co-insurance provisions.
A Health Maintenance Organization (HMO) provides a wide range of healthcare services to you on a prepaid basis. Under this plan, you receive medical services at no cost or for moderate co-payments without deductibles or claim forms.
In 2016, we are excited to offer those eligible employees the MDC Jackson First HMO Plan Option as as additional option to your current healthcare choices. Also, effective January 1, 2016 - Bariatric services will be covered under the offered HMO plans. This added coverage will be limited to Jackson facilities/providers only. Previously, bariatric services were only covered under the POS plan. Must meet specific criteria to qualify for coverage. Please contact AvMed at 800-682-8633 for the specific criteria that needs to be met in order to qualify for coverage and for any further details about your coverage.

Effective January 1, 2015, our new Pharmacy Benefits Manager (PBM) is CVS Caremark.
This change will result in savings to our medical plan without affecting the pharmacy network and formularies. Your new AvMed member ID cards will indicate a new BIN (Bank Identification Number) to facilitate proper claim processing at retail pharmacies.
Important Note: Any mail order prescriptions must be on the new CVS Caremark mail order form.

The Select Plan
MDC Select HMO
The MDC Select HMO is a plan option that was first offered in 2015 to non-bargaining unit employees and to eligible bargaining unit employees per their respective Collective Bargaining Agreement, for employees living in Miami-Dade, Broward, or Palm Beach Counties.
US Imaging joined Miami-Dade County’s South Florida Provider Network for all health plan options.
US Imaging offers a unique VIP radiology program for outpatient advanced imaging services such as CT, MRI and PET scans through a Network of high quality imaging Providers. Participation is voluntary and you do not have to accept the offer from US Imaging or go to the suggested facility.
You can have the procedure at any of the other participating imaging facilities offered in your Miami-Dade County health plan.
Members of the DCFF fire union may be eligible for coverage in their Union-sponsored plan. Contact your Union office for further details. If you enroll in the Fire Union-sponsored health plan, you may elect the Optix vision plan, but you cannot participate in of any County-sponsored dental plans.
