COVID-19 Vaccine and Booster Shot Finder

Individuals 18 years of age or older who live in the United States may receive the COVID-19 vaccine for free in Miami-Dade County. Proof of Florida residency is not a requirement to receive the vaccine in Florida but individuals must provide a photo identification. Minors between the ages of 6 months – 17 years of age may receive the vaccine but must be accompanied by a parent or guardian and bring with them a completed parental consent form.  For the latest COVID-19 Vaccine guidelines, please visit www.cdc.gov.

Effective July 1, 2022, individuals without a domestic (U.S.) address and no health insurance will be charged $40 for vaccine administration at the County-sponsored locations managed by Nomi Health.

Acceptable forms of photo identification include:

• Florida driver license
• Florida identification card issued by the Department of Highway Safety and Motor Vehicles
• Passport
• Consulate ID
• Employee ID
• Farm Worker or Nursery Worker ID
• Debit or credit card
• Military identification
• Student identification
• Retirement center identification
• Neighborhood association identification
• Public assistance identification
• Veteran health identification card issued by the United States Department of Veterans Affairs
• A license to carry a concealed weapon or firearm issued pursuant to Florida Statute 790.06

The Centers for Disease Control and Prevention (CDC) recommends that individuals 18 years of age or older get a booster shot of the Pfizer-BioNTech or Moderna COVID-19 vaccines. The CDC also recommends that minors 5-17 years old receive a booster of the Pfizer vaccine. A booster shot can be provided at least five months after completing the primary Pfizer or Moderna vaccination series. Individuals may choose which COVID-19 vaccine to receive as a booster shot. A booster shot of the Johnson & Johnson COVID-19 vaccine after two months is also recommended for those who are 18 and older. 

On March 29, 2022, the CDC expanded eligibility for an additional booster dose for certain individuals who may be at higher risk of severe outcomes from COVID-19. People over the age of 50 can get an additional booster 4 months after their prior dose to increase their protection further. This is especially important for those 65 and older and those 50 and older with underlying medical conditions that increase their risk for severe disease from COVID-19.

People eligible to receive the booster shot can visit any County-supported vaccination sites being operated by NOMI Health as well as CVS Pharmacy or Walgreens Pharmacy. Bring a valid photo ID and proof of receiving the initial doses of the vaccine.

Residents may use ride-sharing car services likes Uber or Lyft to arrive at a vaccination site. Please ask the driver to alert the staff upon arrival at the drive-through vaccination location that the patient is arriving by use of a ride-sharing car service.

Florida Medicaid provides transportation to Medicaid recipients to get the COVID-19 vaccine at no cost. Recipients should set up a time to get their vaccine and let their Medicaid plan know they need a ride. Recipients who are not enrolled in a plan can call the Medicaid Helpline at 1-877-254-1055 to find out the name and phone number for a transportation service. If you lost your CDC COVID-19 vaccination card and obtained the vaccine at one of the State of Florida vaccination sites, you may submit a request to the Florida SHOTS (State Health Online Tracking System) Program. The Florida SHOTS Program is a free, statewide, centralized online immunization registry that assists health care providers, schools and parents with keeping track of immunization records. All COVID-19 vaccination sites must submit information on the patients they vaccinated to Florida SHOTS.
