Personal Injury Protection Medical Providers License

Medical providers that provide medical or physical therapy treatment to patients claiming injury from an automobile accident must register with Miami-Dade County.
Providers must also meet one of two financial guidelines:
90 percent more of its monthly gross income from billing Personal Injury Protection Insurance Providers; or
$200,000 or more in gross income during a 12-month period from the billing of Personal Injury Protection Insurance Providers.
Completed application packages can be submitted by mail, in person, or online. If applying online, make sure to follow these instructions.
Copy of Florida driver’s license or government issued identification for each owner and all other persons identified in the application
Copy of each active State of Florida license for persons listed on the application including:
Medicine pursuant to Chapter 458, Florida Statutes
Osteopathic medicine pursuant to Chapter 459, Florida Statutes
Chiropractic medicine pursuant to Chapter 460, Florida Statutes
Dentistry pursuant to Chapter 466, Florida Statutes
Physical Therapy pursuant to Chapter 486, Florida Statutes
Acupuncture pursuant to Chapter 457, Florida Statutes
Massage Therapy pursuant to Chapter 480, Florida Statutes
If applicable, a copy of the Health Care Clinic License or Exemption issued by the Florida Agency for Health Care Administration or Florida Department of Health
Copy of the Certificate of Occupancy issued by Miami-Dade County, or from the municipality where the provider is located
Floor plan of the provider showing all areas, including the location of controlled substances
Sworn and notarized Owner Affidavit for each owner (part of the application)
Sworn and notarized Designated Physician Affidavit (form attached to the application)
How to apply:
Any person(s) or corporation that operates a pain management clinic business in Miami-Dade County must fill out a permit and/or license, submitted and approved by Miami-Dade County by applying online, in person or by mail.
If the application process is not completed within 60 days of its filing, the application will be considered abandoned and closed and a new application and fee will be required.
Annual Personal Injury Protection Medical provider registration: $535.00
Late registration fee: $267.50
Replacement certificate (optional): $20.00
NOTE: Effective July 1, 2023, Section 163.211, Florida Statutes will preempt the local licensing of Personal Injury Protection Medical Providers. As a result, applicants seeking a new or renewal registration will be issued a registration with a July 1, 2023 expiration date. The fees will be prorated on a monthly basis to cover the period between the month of issue and July 1, 2023.
The fees are non-refundable. A late fee will apply to renewal applications submitted after the expiration of the current registration. The late fee is in addition to the renewal fee.
Checks or money orders should be made payable to Miami-Dade County / CP. Visa, Mastercard and American Express are accepted when applying online and in person.
Medical providers are exempt from registering if:
Licensed as a facility pursuant to Chapter 395, Florida Statutes
The majority of the physicians, who provide services in the facility, primarily provide surgical services for the facility;
Owned by a publicly held corporation whose shares are traded on a national exchange or on the over-the-counter market and whose total assets at the end of the corporation’s most recent fiscal quarter exceeded $50 million
Affiliated with an accredited Florida medical school for the education of physicians licensed under Chapters 458, 459 or 460 of the Florida Statutes, at which training is provided for medical osteopathic, or chiropractic students, residents or fellows
Owned by a corporate entity exempt from federal taxation under U.S.C. 501(c)(3) or (4)
Perform only magnetic resonance imaging, static radiographs, computed tomography or position emission tomography; or
Accredited by the Joint Commission as an Outpatient/Ambulatory Care clinic
