Premise Location Certificate

• To order a certificate, complete the Premise Location Certificate Request form and submit it by fax, mail or in person.
• To request the process by fax, a Premise Location Certificate account must be established and payment prepaid.

Fax your request to establish an account and provide a telephone contact so a department representative can reach you.

• Advance payment of $20 per request for a 24-hour written response.
• Advance payment of $10 per request for a written response within 7 to 10 days.

To pay:

• Establish a Premise Location Certificate account. Payments can then be drawn from that account if you make numerous requests.
• Mail the request, payment, form and self-addressed stamped envelope.

Properties not served by the Miami-Dade Water and Sewer Department will be advised accordingly on each Premise Location Certificate. However, the Department does not provide written notice of service verification through the certificate process.
