Applications can be submitted online or in person at the Golden Passport Office or at any 311 service center.

When applying online or in person you will need to submit:

• Medicare card
• A current valid US state-issued identification

The easiest way to replace your lost, stolen, or damaged Discount Fare EASY Card for Medicare Recipients is online.

Replacement fees:

• $5 for the first replacement
• $20 for the second replacement
• $50 for each replacement thereafter

A fee waiver is available when an original police report is presented, listing the card as a stolen item.
